The region is administratively managed through a local government system, divided into 15 Municipal and District Assemblies. These consist of 4 Municipal and 11 Ordinary Assemblies. Each District, Municipal, or Metropolitan Assembly has a Chief Executive, representing the central government, with authority drawn from an Assembly led by a presiding member chosen from among its own members.

The 15 MMDAs encompass areas like Bawku, Bawku West, Binduri, Bolgatanga, Bolgatanga East, Bongo, Builsa North, and Builsa South, along with Garu, Kassena Nankana East, Kassena-Nankana West, Nabdam, Pusiga, Talensi, and Tempane.
